How to use the questionnaire on paper

Print as many questionnaires as there are people in the draw.
Hand them out before the draw: everyone writes their name and answers – for a child, a parent does.
Collect them all, folded, in one envelope or box.
After the draw, everyone gets the questionnaire with the name they drew.
